In the Krasnoyarsk Territory, they plan to revise the system of providing the population with preferential medicines. This issue was discussed at the Committee on Health and Social Policy of the Legislative Assembly.
The results of checking the efficiency of expenditure of funds allocated to the Ministry of Health of the region for medicines to certain categories of citizens were summed up by representatives of the Accounts Chamber of the region. The Chairman of the Committee Ilya Zaitsev noted that providing preferential medicines is the most important indicator of accessibility and quality of medical care, it is under constant control of the committee.
The auditor of the Accounts Chamber Larisa Titova noted that representatives of the ministry did not provide all the information necessary for analysis, did not have enough data to provide children under 3 years of age, oncopacients, palliative patients and people living in rural areas.
In total, more than 1.1 million inhabitants of the region have the right to preferential drug support. Within the framework of the state program of the Territory “Health Development”, 20 measures are being implemented providing for drug support. At the same time, about 390 thousand people, or 33% of those who have the right to this measure of support, were able to use their right.
Analysts believe that problems are not in the absence of finances, but in the imperfection of the regulation of this issue. The ministry does not conduct system work with administrative documents. There are questions to the procedure for planning in the formation of the regional budget: there are no calculations of the need for the volume of drugs and funds for their acquisition, which contradicts the norms recommended by the Ministry of Health of the Russian Federation.
Moreover, the actual budget expenditures for these purposes are growing. So, in 2024, financing of drug support amounted to about 9 billion rubles, which is twice as much as in 2021.
A separate issue is the expenses of the regional budget for the drug support of federal beneficiaries. The profile committee has repeatedly designated this problem as unbearable for many regional budgets at the highest federal venues, including the performance of the speaker of the regional parliament Alexei Dodatko on the Council of Legislators of the Russian Federation.
In 2024, immunobiological preparations were acquired 30% less need, funds for tuberculinodiagnosis of children – by 40%. Since 2023, medical organizations have not been provided with vaccine against tick -borne encephalitis for socially unprotected segments of the population, which creates additional risks of increasing incidence of tuberculosis and tick -borne viral encephalitis.

Summing up the discussion, Ilya Zaitsev thanked the control department for the work: “The report shows the difficulties that are in the system of preferential drug support. We understand that the problem cannot be solved simply by an increase in funds, the gap between the planned application and the actual vacation of drugs is obvious. Unfortunately, we state difficulties with planned work: there is no altitude register of patients whose treatment requires significant investments: expensive drugs, special nutrition. The Krasnoyarsk Territory can be proud of both the amount of expenses and our financial capabilities for preferential medicines, we need to improve the system. ”
The Committee proposes to attract an external medical and economic audit of one of the leading Russian institutions for the analysis of the most costly nosologies. Among the proposals for changing approaches to solving a vital issue – to make an annual public report on issues of preferential drug support. It will also be proposed to include a control event to evaluate the effectiveness of drug changes to the work plan.
