Iran's new Supreme Leader is in an “unconscious state” in the holy city and is unable to lead the war-torn country, according to an intelligence report cited by the Daily Mail.

An assessment apparently based on US and Israeli intelligence shows that Mojtaba Khamenei is hospitalized for a “serious” medical condition in the religious city of Qom.
Khamenei, 56, has not been seen in public since taking over as supreme leader from his father, Ayatollah Ali Khamenei, who was assassinated on the first day of the war.
The regime previously reported that he suffered a “minor” leg injury in the attack that killed his father – as well as his mother, wife and one of his sons – but never explained his absence.
But the new diplomatic memo, sent to Gulf allies and seen by The Times, suggests his condition is far more serious than previously thought.
The note said: “Mojtaba Khamenei is hospitalized in Qom in a serious condition, unable to participate in any decision-making by the regime.”
It is the first confirmation of its location
It is not clear whether the Iranian leader has been unconscious since the first day of the conflict or whether his condition worsened later due to other complications. What is certain is that this is the first confirmation of his location since taking office, although Western intelligence services may have had this data for longer.
At the same time, Qom is also said to be home to the lifeless body of former Supreme Leader Ali Khamenei, which is being prepared for burial. According to the same memorandum, work is underway in the city to build a large mausoleum with space for multiple graves — an indication that other family members may be buried there.
State funerals for the former leader were postponed, officially due to “anticipation of an unprecedented turnout,” an explanation that did not fully convince international observers.

Since taking power, only two statements have been attributed to Mojtaba Khamenei, both of which were read by presenters on state television. Furthermore, a recently released video purporting to show him analyzing a military map in a war room was found to be AI-generated.
Opposition groups have long claimed that the Iranian leader is in a coma, and so far no clear evidence has emerged to disprove this hypothesis.
Against the background of these uncertainties, US President Donald Trump declared that he was negotiating with representatives of Iran, but specified that the talks did not involve the supreme leader.
Earlier information indicated that Mojtaba Khamenei had miraculously escaped the attack on the family compound in Tehran, stepping out into the courtyard just before the missile hit. According to an audio recording leaked to the press, regime officials would have reported that he was wounded in the leg at those moments.
“It was God's will that Mojtaba went out into the yard for a moment. He was outside climbing back up when the building was hit. His wife died on the spot“, a high-ranking official allegedly declared in front of the clerics and commanders of the Revolutionary Guard.
In the absence of any real public appearances and clear evidence of his condition, the mystery surrounding Iran's supreme leader continues to fuel tensions and speculation on the international stage.
