Doctors of the Krasnoyarsk hospital No. 20 operated on the patient with the risk of re -stroke

In Krasnoyarsk, the doctors of the 20th hospital for the first time underwent surgery on the vessels of the patient’s neck with a risk of re-stroke. This was reported in the regional Ministry of Health.

Operations on a carotid pool are one of the most difficult in vascular surgery, because speed and marginal accuracy are important here, any mistake can lead to a stroke and death of the patient. The woman 53 years old was sent to the surgeons by the neurologist of the cabinet of the secondary prevention of the stroke of the vascular center of the Krasnoyarsk inter-district clinical hospital No. 20. Earlier, she had already suffered a stroke, after which there was a residual neurological deficit: complaints about periodic dizziness, a sense of noise in her head and ears, awkwardness when walking and other neurological defects.

“The patient had an occlusion of the internal carotid artery on both sides, and the brain fed due to only the vertebral arteries. The risk of repeated acute cerebral circulation disorders was very high, and the outcome could be unfavorable. Endovascular intervention in its case could not be performed due to the features of the anatomy – the cooler of the vessels. It was decided to perform an open intervention, ”said Alexander Tolstikov, head of the surgical department No. 2 of the KMKB No. 20.

The woman was made of carotid endarctomy of the general and internal carotid artery. In order not to stop the blood flow for a long time and prevent the death of the brain, the patient performed temporary shunting of blood flow with a carotid shunt. Further, the surgeons removed the atherosclerotic plaque and the arteries sewed a synthetic patch in place of the defect.

“On average, such operations last 2 hours. Given the complex anatomy and technical features of the structure of the artery, we did everything quite quickly – in 2 hours 15 minutes. After the operation, the patient was woke up on the table. No neurological violations were revealed. She spent a day in the resuscitation department, then a week in a regular ward. He feels perfectly, I am satisfied with the result of the operation, prepares for an extract. Then it will be observed in the clinic at the place of residence, ”the surgeon explained.

Ahead of the patient is an important stage: strict compliance with the diet, labor and rest, continuation of the neurorebilitation program and taking prescribed drugs. This will help maintain the vascular reconstruction zone and avoid the risk of repeated stenosis of the carotid artery and stroke.

Recall that earlier such operations are already performed at the Federal Center for cardiovascular surgery, a regional clinical hospital, and KMKBSMP named after N.S. Karpovich, and now they will be performed in the 20th hospital of Krasnoyarsk.
